As always, at the Axiom Dynamics Training Academy, we will continuously strive to make things less complicated for our learners to achieve success in whatever qualification they are studying. With this in mind, we have put together a simple checklist for the Level 4 Certificate in Leading the Internal Quality Assurance of Assessment Processes and Practice.

Within your Level 4 Certificate in Leading the Internal Quality Assurance (IQA) of Assessment Processes and Practice portfolio, there are folders for Units 1, 2 and 3, a Course Administration folder, and your Learner Portfolio. (This is the folder that contains all information about your learners). This checklist should be used before submitting these units to your course tutor, as it will ensure you have all documents completed.

Recognised Prior Learning

Please upload any relevant certificates. These can be used as recognised prior learning (RPL), which means you will not need to complete those units again.    

ID Upload

This is your proof of identification and address to register you as a learner and to submit your portfolio for marking.  

CV Upload

Copy of your current CV.

Candidate Assessor Details

Both ‘Your Details’ and the ‘Initial Assessment’ need completed here.      

Achievement Summary Sheet

You will need completion dates entered in the Level 4 Certificate in Leading the Internal Quality Assurance of Assessment Processes and Practice for all three units.

Specimen Signature Sheet

Your signature and that of anyone else involved in the process.   

Assessment and Action Plan

This form is what you complete when planning what each learner must achieve during the assessment visit. You will need 4 of these completed for the entire Level 4 Certificate in Leading the Internal Quality Assurance of Assessment Processes and Practice course, (2 per learner for Unit 2 and 2 per learner for Unit 3).       

Assessment Records

This document is to be used by you during your assessments to record all the decisions you make. You will need 8 of these completed for the entire Level 4 Certificate in Leading the Internal Quality Assurance of Assessment Processes and Practice course, (2 per learner for Unit 2 and 2 per learner for Unit 3).        

Evidence Tracking Sheets

These evidence sheets are provided to help you keep track of your progress in completing your portfolio.           

 

Unit 2 and 3 Checklist                                            

Folder

Unit 2 Workbook

Each Learning Outcome answered should be answered accordingly. This can be a reference point to another folder within your portfolio, demonstrating this learning outcome or additional information added to complete your evidence of understanding. 

Unit 3 Workbook

Each Learning Outcome answered should be answered accordingly. This can be a reference point to another folder within your portfolio, demonstrating this learning outcome or additional information added to complete your evidence of understanding. 

Individual Learning Plan(s)

Signed off and dated upon completion of each Learning Outcome by yourself and for both Unit 2 and Unit 3. Your own tutor/ assessor will then sign this when submitted.      

Personal Profile

The documentation which is applicable to you, such as:

  • CV
  • Personal profile (describing role, where you are located, how long you have been with your workplace/organisation and your aspirations)
  • Relevant certificates

Assessor Profiles

A profile of the 2 assessors that you will be carrying out all IQA activities with. This upload area is to include documentation applicable to them, such as:

  • CV
  • Personal profile (describing their role, where they are located, how long they have been at their workplace/organisation)
  • All relevant certificates to assess their vocational or workplace subject

Observation Plan

The yearly plan you will use to show which assessors will be observed, including dates, times and location.           

Team Meeting Plan

A yearly plan to show when team meetings will take place including dates, times and location.

Standardisation Plan

Your yearly plan to show when standardisation activity meetings will take place including dates, times and locations.      

Sample Plan

A sample of assessed work that has been internally quality assured.

This needs to include 2x plans, 1 for each assessor to cover a cross section of work from different learners, locations, assessment methods and assessment decisions.     

IQA of Assessment Document

This is the document/s which is used as part of the assessment to ensure all criteria/requirements are met/covered. This should be a scanned copy of the original documentation, eg. a leaner’s workbook or a multiple-choice assessment.

IQA of Learner Work

This is a sample of learner’s work taken from your sample plan. This should be a scanned copy of the original documentation.       

IQA Observation of Assessors

These coincide with your observation plan.

A qualified TAQA assessor should be with you on your second assessment to compile this report as evidence.

This will also include your Learner Discussion Checklist where you meet with the learner after the assessment has taken place.

Learner Discussion Checklist

This is where you will talk to your assessors’ learners and gain their feedback regarding the assessment process.        

Professional Discussion with your Assessors

This is completed with your assessor on each your visits.

Team Meeting Minutes

A sample of minutes of meetings, taken from your Team Meeting Plan.  

Standardisation Activities

A sample of minutes of meetings taken from your Standardisation Activities Plan eg. discussing qualification updates, new policies, induction material etc.

Policies

A minimum of 3 policies that are used within your organisation eg. health & safety, equality and diversity, data protection/ GDPR.        

Reflective Practice

Evidence of reflective practice in the form of a reflective learning journal, self-evaluation form or SWOT analysis on yourself.

Personal Development Plan

A document planning your development and continuing professional development (CPD) activities.    

Continuing professional development (CPD)

CPD can be anything that you do that helps you improve your practice.   

Supporting Documents

This area is to be used to upload any supporting documents, such as:

  • Qualification Specifications
  • Standards
  • Criteria
